Prior to your first visit we advise you to call your insurance company\nto confirm that we are within your specific insurance network, to determine\nyour copay \/ coinsurance liabilities and to determine if a referral from your\nprimary physician is required. You usually can find the number to call on your\ninsurance card. It is extremely important for you to know your coverage and it\nis your responsibility to understand whether your insurance has limits on the\ndoctors you can see, the services you can receive, and your copay \/ coinsurance\nliabilities. <\/p>\n\n\n<div class=\"wp-block-image\">\n<figure class=\"alignright size-medium\"><img decoding=\"async\" width=\"768\" height=\"512\" src=\"https:\/\/www.entofathens.com\/wp-content\/uploads\/2019\/09\/ThinkstockPhotos-83405702-768x512.jpg\" alt=\"\" class=\"wp-image-1410\" srcset=\"https:\/\/www.entofathens.com\/wp-content\/uploads\/2019\/09\/ThinkstockPhotos-83405702-768x512.jpg 768w, https:\/\/www.entofathens.com\/wp-content\/uploads\/2019\/09\/ThinkstockPhotos-83405702-1280x853.jpg 1280w, https:\/\/www.entofathens.com\/wp-content\/uploads\/2019\/09\/ThinkstockPhotos-83405702.jpg 2048w\" sizes=\"(max-width: 768px) 100vw, 768px\" \/><\/figure><\/div>\n\n\n<p>We are contracted with most insurance companies, including,\nbut not limited to:<\/p>\n\n\n\n<ul>\n<li>Aetna<\/li>\n\n\n\n<li>Blue Cross Blue Shield<\/li>\n\n\n\n<li>Cigna<\/li>\n\n\n\n<li>Humana<\/li>\n\n\n\n<li>United Healthcare<\/li>\n\n\n\n<li>Medicare<\/li>\n\n\n\n<li>Medicaid of Georgia<\/li>\n\n\n\n<li>Coventry <\/li>\n<\/ul>\n\n\n\n<p>If you do not see your health insurance on our list, please call your health insurance and\/or our office at <strong><a href=\"tel:+17065467908\">706-546-7908<\/a><\/strong> to determine if we are an in-network provider. <\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Diagnostic Procedures <\/h2>\n\n\n\n<p>Please be aware that when a patient requires a visit to a\nspecialist, there are sometimes diagnostic procedures required to diagnosis and\nevaluate for treatment that cannot be done by a primary care provider. These\nprocedures may be done during the normal course of the exam by the specialist.\nDepending on your insurance policy provisions, these procedures and others may\nfall under a separate benefit other than your office copay, such as a\ndeductible or co-insurance. <\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Managed Care Referrals <\/h2>\n\n\n\n<p><strong>Most\npatients to ENT of Athens do not require referral by your primary care\nphysician.<\/strong><\/p>\n\n\n\n<p>However, if you have an HMO or similar plan, you will need a\nreferral from your primary care physician to see a specialist. If we have not\nreceived this referral prior to your arrival at our office, your appointment\nwill be rescheduled. It is your responsibility to know if a referral is\nrequired and to obtain one. If one is not obtained, you will unfortunately be\nresponsible for the entire bill. <\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Financial <\/h2>\n\n\n\n<p>If you provide complete and accurate information about your\ninsurance, we will submit claims to your insurance carrier and receive payments\nfor services. Depending on your insurance coverage, you may be responsible for\nco-payments, co-insurance, or other deductible amounts WHICH ARE DUE AT THE\nTIME SERVICE IS RENDERED. We accept cash, check, VISA, MASTERCARD, DISCOVER and\nAMERICAN EXPRESS. Payment plans must be arranged on an individual basis with\nthe Billing Department. If you have any questions, please review the financial\nform included in your registration packet or contact our billing department at\n706-510-0486.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Self-Pay Patients <\/h2>\n\n\n\n<p>Our office requires patients without insurance to pay in\ncash or with a credit card on the date of service. If you have any questions or\nconcerns, please contact our office at 706-510-0486. <\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Health Plan Participation <\/h2>\n\n\n\n<p>We participate with most major health plans in the area. We\nadvise you to call your insurance company to confirm that we are within your\nspecific insurance network.
